Perimenopause is the term used to describe the transitional period that leads to menopause, which is when menstrual periods stop. The perimenopause phase typically begins in the mid-40s but it is not uncommon for it to start in the late 30s. It begins when the ovaries gradually decrease their production of estrogen, progesterone and testosterone and on average, lasts 4 years, but can last up to 10 years. Perimenopause is typically diagnosed based on clinical symptoms and reported physiological changes, but hormonal lab tests to rule out other medical issues that could be contributing to a woman’s symptoms. Bioidentical hormone replacement (BHRT) can be helpful for alleviation of symptoms and overall health, when appropriate.

1. Menstrual Cycle (“Period”) Irregularities

• Altered cycle length: Periods last longer or shorter than usual and are hard to predict.

• Skipped cycles: Periods are missed for 60 days or more without another explanation.

• Variable flow: Menstrual flow (bleeding) becomes significantly heavier or lighter.

2. Vasomotor Symptoms

• Hot flashes: Sudden, intense sensations of heat centered on the face, neck, and chest, often accompanied by flushing and rapid heart rates.

• Night sweats: Severe episodes of sweating during sleep. These are disruptive to sleep and many women need to get up to change their clothes.

3. Mental and mood Changes

• Mood disturbances: Worsening irritability, anxiety, and depressive symptoms can be triggered by fluctuating estrogen levels.

• Cognitive difficulties: “Brain fog”, difficulty concentrating and short-term memory difficulties are common.

• Sleep disruption: Sleeping becomes harder than usual; some women have trouble falling asleep and/or staying asleep. This can be from night sweats or from the effects of chaotic hormone levels.

4. Metabolic and Physical Signs

• Body composition changes: Metabolism slows and abdominal fat increases. Losing weight often becomes harder even when diet and exercise are increased (or haven’t changed from what previously worked to keep fat off.)

• Urogenital symptoms: Genitourinary syndrome of menopause begins for ~80% of women. Decreased estrogen can lead to vaginal dryness and thinning of vaginal tissues, which may cause discomfort during intercourse or recurrent urinary issues.

Diagnostic and Clinical Evaluation

There is no single blood test that can definitively confirm perimenopause. Oftentimes, a healthcare provider will check Follicle-Stimulating Hormone (FSH) and thyroid levels to rule out other medical conditions, but an accurate diagnosis is based on your symptom pattern. This is why it’s important to work with a clinician who understands perimenopause and it’s clinical presentation. Evidence-based interventions, including bio identical hormone replacement therapy (BHRT) and targeted lifestyle modifications, are available to manage this transition effectively.

Clinical Consultation

Self-diagnosis based solely on generalized fatigue or weight gain can be highly inaccurate. It is important to be evaluated by a knowledgable clinician who can assess your entire medical picture, diagnose and treat perimenopause (if appropriate), as well as make other recommendations for optimal health.

Perimenopause is inevitable but suffering is not.
